Where does this data go? Reports you submit become part of the patient's longitudinal record and are shared with other clinicians only under the patient's consent. A de-identified, aggregated copy also flows to national reporting (DHIS2/NHMIS, MSDAT) — original identifiers never leave NHCIP.
